Belohlavek, Jason Doncese 41 on January 30, 2017 after a lengthy battle with heart disease. Jason was the loving son of Judy and Michael and beloved brother of Adeline Phelps. He is survived by his nephew and Godson, Aidan, his niece, Anna and brother-in-law, Matthew Phelps. Jason loved deeply and was deeply loved. He will be missed beyond words.visitation Monday February 6th 3-5 and 7-9 pm Mass at the Church of St Patrick Tuesday 10 am. Interment Holy Rood Cemetery, Westbury N. Y.
